Nurse Practitioner

Hinds Hospice, one of the most well-known and well-respected nonprofit agencies in California's Central Valley and a pioneer in the practice of hospice and palliative care, is seeking a Palliative Care Nurse Practitioner to join our team.
The role of the Palliative Care Nurse Practitioner is to provide palliative care for identified Hinds Palliative patients in patients homes, ALF, and SNF. Consult and assist with the referring physician, the acute care team, and other members of interdisciplinary team to coordinate the patient's plan of care. Provide consultation in palliative care, symptom management and supportive care to meet the needs of the patient as requested by the referring physician.
Prepare and maintain accurate patient records, charts and documents to support the medical practice and reimbursement for services provided
Establish relationships with existing hospital-based palliative care teams, partner companies, medical directors, hospital physicians, community physicians, nurses and other team members as well as community members to build a culture of collaboration and clinically integrated care across the continuum
Proactively identify opportunities to improve the patient and family experience of care and to improve the efficiency and effectiveness of resource use throughout Hinds
Develop outreach programs for community agencies and health care settings (such as nursing homes), as appropriate. Educate and assist Hinds team members on End of Life/Palliative Care. Participate in community outreach presentations to interested groups or community advisory committees at each hospital site
Participate in biweekly Palliative Care team meetings to review active cases
Participate in monthly and periodic team meetings for the operational and administrative functions of the Palliative Program
Be available on an as needed basis to support other team members with clinical advice and assist with physician communication needs
Be available on an as needed basis to support educational and outreach goals of the Hinds Palliative Program
Act as spokesperson for Hinds Palliative

Qualifications for this position include but are not limited to:
Completion of a professional nursing program accredited by the National League for Nursing.
Completion of Masters Program in nursing. Current NP Licensure in the state of CA.
Demonstrated ability to work with health team personnel.
At least 2 years of experience in the hospice/palliative care setting (prefer one-year hospice experience).
Recommended (not required) to have certification in hospice and palliative nursing.
Able to function autonomously with flexibility and confidence, and possess good time management skills.
Must have a valid drivers license and a willingness to use personal vehicle in the course of employment.
Must show proof of automobile insurance, proof of current auto registration and acceptable driving record (no more than 3 points within a 36-month period).
Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 35 lbs.)

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
